Francesca Higgins
Background
After struggling with recurring knee pain during long gravel races, Francesca turned to Foot Factor for a full assessment. This made even training rides too uncomfortable to enjoy, and put a temporary halt to her riding.
She reported:
- Pain to the medial and anterior aspects of the left knee
- A feeling of instability and malalignment on her bike
- Struggling to find comfort in her bike shoes
After working with her physio and bike fitter, she came to Foot Factor for a biomechanical assessment.

Assessment
Her biomechanical assessment revealed:
- A flexible pronated foot posture, causing instability within the shoe
- Uneven plantar pressure distribution
- A valgus position of the knee during pedal stroke
Intervention
Her bike fitter had already adjusted her cleats correctly and suggested a well-fitting shoe. She had started her rehab with her physiotherapist. She was prescribed bespoke foot orthoses designed specifically for cycling designed with:
- A forefoot varus wedge and medial arch support, which helped to align the knee and lower leg on the bike and to aid in power transfer
- A close fit with the arch and foot contours to distribute pressure and stabilize the foot within the shoe
- A stiff shell and lightweight construction to complement her bike and shoe setup and optimize efficiency
Results
- Immediate improvement in foot comfort and shoe fit
- Improved alignment of the lower leg and knee
- A significant reduction in knee pain
- A return to training and racing
Conclusion
Through advanced analysis and bespoke cycling orthotics tailored to her riding style, we helped her stabilise her pedal stroke and eliminate the discomfort. Just weeks later, Francesca completed a 100km gravel event pain-free—with her best time yet.
